New Patient Registration

We welcome new patients living within the areas listed below, under Practice Area.  Please Telephone Kingswells reception on 01224 745800 between 8am - 5pm,  Monday - Friday, and the receptionist will then make a registration appointment with either the GP and/or the Practice Nurse if appropriate. 

This appointment is to obtain a detailed clinical history as well as to obtain basic health measurements including health data, blood pressure reading and urinalysis. 

*In addition to the Practice Registration Form, you are required to complete a patient questionnaire for each member of the family who is registering with the practice.

When you move out of the practice area, you will need to register with a new practice as soon as possible.

Please email the appropriate health questionnaire to gram.kingswellsreception@nhs.scot.  Your registration will not be accepted if you live outwith the Practice boundary.

Preference of Practitioner

The doctors work in partnership as a group, and your registration will be with the practice rather than with an individual doctor.  You may consult with any of the doctors in the practice, although it is suggested that you stay with the same doctor for a particular problem. Male and female doctors are available and you may request an appointment with either. 

Should you only ever wish to see a particular doctor we require this request to be made in writing, but please bear in mind that this may necessitate a longer wait for an appointment.

PLEASE NOTE THAT 7 VICTORIA STREET, ABERDEEN, IS NOW CLOSED.  ALL SERVICES ARE NOW PROVIDED FROM OUR KINGSWELLS SITE ONLY.

Kingswells Medical Practice offers patients the opportunity to register for Patient on-line Services (www.patient-services.co.uk).   This will enable you to make a doctor's appointment, cancel doctor or nurse appointments and to check when your appointment is, should you forget.    Patient Services also allows you to order your repeat prescription medication on-line.

If you wish to register for the above services, please complete the Online Patient Registration Form below and bring this along to Reception with photographic I.D. and proof of address.  Patients who are under 14 years of age can use their parent's email address.  However, we would ask that once patients achieve their 14th birthday that they register with their own email address due to issues surrounding patient confidentiality.
