New Patient Registration

We welcome new patients living within the areas listed below, under Practice Area.  Please call at the Kingswells reception desk and the receptionist will then make a registration appointment with either the GP and/or the Practice Nurse as appropriate. 

This appointment is to obtain a detailed clinical history as well as to obtain basic health measurements including health data, blood pressure reading and urinalysis. 

*In addition to the Practice Registration Form, you are required to complete a patient questionnaire for each member of the family who is registering with the practice. If you have a medical card please bring it with you. When you move out of the practice area, you will need to register with a new practice as soon as possible.

Practice Area

Our practice area is within a boundary enclosed by the following streets: 

Practice boundary as of 5 August 2019:  From City boundary with Westhill on the A944, heading south down to Kingshill  (on the back road), heading East on Countesswells Road, heading North and crossing over Hazledean Road, North on Groats Road, crossing Skene Road and crossing Lang Stracht (just West of Sheddocksley).   Cut across to Howes Road, then Northbound on the A96 (Inverurie Road) until it meets the B979 at the top of Tyrebagger and West on the B979, until it meets the City boundary at Westhill.

Preference of Practitioner

The doctors work in partnership as a group, and your registration will be with the practice rather than with an individual doctor.  You may consult with any of the doctors in the practice, although it is suggested that you stay with the same doctor for a particular problem. Male and female doctors are available and you may request an appointment with either. 

Should you only ever wish to see a particular doctor we require this request to be made in writing, but please bear in mind that this may necessitate a longer wait for an appointment.
